Eligibility: Players must be at least 16 years of age to participate and must have a parent/guardian fill out and sign a release form if under 18 (found below).

Registration Instructions: Please fill out the registration and roster forms or click the link for online registration. Email the forms to recreation@meridiancity.org or drop-off in person at Meridian City Hall 33 E Broadway Ave, Meridian, ID 83646. Once we have the form, you can call 208-888-3579 and we can take payment over the phone.
